Privacy & Consent Form

Hayley is committed to maintaining your privacy and confidentiality during our sessions. All information shared will be treated with the utmost respect and will remain strictly confidential. However, there are circumstances under which confidentiality may need to be breached:

• If you disclose information indicating you pose a risk to yourself or others, steps may need to be taken to ensure safety.

• Throughout and following our sessions, Hayley will maintain handwritten notes documenting the topics discussed and progress made. These notes will include personal details such as your name, address, contact information, and date of birth. They will be securely stored for five years for insurance purposes, and you may request access to them during this time. In the event of Hayley’s inability to safeguard these notes, trusted individuals have been designated to ensure their secure disposal.

By engaging in sessions with Hayley, you consent to the following terms:

• Recording of Personal Data: Hayley may manually record your personal data and session details.

• Access to Records: You have the right to view and request copies of your records within five years of the session date.

• Communication: Hayley may contact you via phone, text, email, or online.

• Withdrawal of Consent: You retain the right to withdraw consent at any time by providing written notice to Hayley.

Consent Agreement for EFT and Matrix Reimprinting Sessions:

It is important to understand that EFT and Matrix Reimprinting sessions are not substitutes for medical, psychological, or psychiatric treatment. While they complement traditional healing practices, they are not intended to replace professional medical advice, diagnosis, or treatment. By participating in sessions with Hayley, you agree to the following:

• Acknowledgment of Complementary Nature: EFT and Matrix Reimprinting sessions are complementary to existing medical treatments and meant to enhance overall well-being.

• Continuation of Medical Advice: You will continue to follow the advice and treatment plans prescribed by your medical professionals.

• Responsible Use: You understand the scope of the services provided and agree to use them responsibly.

• Prioritisation of  Health and Safety: Please consult your healthcare provider with any questions or concerns about integrating EFT and Matrix Reimprinting into your health regimen.
